Community Health Worker Program

Community Health Workers (CHWs) play crucial roles in promoting and supporting the health of individuals, families, and communities. They are frontline public health workers who are trusted members of and/or have close understanding of the communities they serve. This relationship allows them to be a link between health/social services and the community, to facilitate access to services, and improve the quality and cultural competence of service delivery. CHWs build trusting relationships with their clients and help to conduct assessments, make referrals, offer health education, navigate complicated systems, and advocate for client’s needs.

Purpose:

To engage and empower members of the community to choose healthy behaviors and make changes that reduce the risk of developing chronic diseases. The Community Health Worker Program host CHW interns, provide training for individuals who are interested in becoming a CHW through our CHW Training Program, and share health information to the community during our annual Health Empowerment Conference.


Community Health Worker Training Program

a) This training course will provide students with the knowledge and skills to work closely with health and human services organizations to promote and create positive social change through the role of a Community Health Worker (CHW).


Community Health Worker Intern

b) Students will be taught how to empower clients, develop practical skills, and support individual development. Students will also learn the importance of outreach and gathering community resources that will guide, educate, and bridge the gap between client and advocate.


Health Empowerment Conference

c) An annual one-day event when subject matter experts are invited to share on health topics that impact the community. Attendees will leave with valuable resources and knowledge of information and become more knowledgeable than ever when they arrived.

Community Health Worker Training Program
